You may use the esignature widgets on the free subscription plan as well. Please take a look at the following guide that can help you with more information on eSignatures.

    Hope this information helps!

is it the same account as the free one or just to complete form, dont really want our patients to have to register with jotforms just to complete the form as they wont do it

    Thanks

    Hi Paul,

If you use the free E-Signature Widget registration won't be necessary to complete your form.

I hope it helps, please let us know if you have any more questions.

    Hi

So even if I subscribe to silver then I would need to have a separate account subscription with docusign or the like if I wanted more than the free form e-signatures ?

Even with Enterprise you can only view the submissions of the user who created them so we couldn't have our contact centre being able to see all patient submissions even with a license each ?

    Thanks

If you want to use DocuSign or Adobe EchoSign on your Jotform, you'll be required to subscribe to their services respectively.

Even with Enterprise you can only view the submissions of the user who created them so we couldn't have our contact centre being able to see all patient submissions even with a license each ?

I am not sure if I understand your query correctly. Do you mean to share the submissions received to your forms with your contact center? If so, it is possible by sharing the submissions page by following guide.

    https://www.jotform.com/help/199-How-to-Share-Form-s-Submissions-Page

However, please note that it is not possible if the account is enabled with HIPAA compliance due to encryption.

If you are referring something different, please let us know. We will be happy to help.

    Thanks!

    Hi

I read the link you sent but it seems to still be very manual, is there any way I can speak to someone to go through our requirements and if using Enterprise could help ?

We want to have forms sent automatically out to patients via an email generated by our CRM as a patients books an appointment, this could be from multiple sources Contact Centre, Online booking etc. Once the form has been completed (Medical Questionaire) we want the Medical Practitioner (PatientAdviser, Nurse, Dietician and Surgeon) who did not send out the form to be able to see the submitted form during the consultation ?

Do you integrate with Dynamics CRM 9.1 its not on your website list as we would like to link the completed forms back to records within the CRM ?

    Thanks

    Hi Paul,

right now we do not have a direct integration with Dynamics. However, you can integrate Jotform with Dynamics through Zapier.

If you would like to look into the Enterprise plan to see if that is something for you, please fill out the form on this link and someone will get back to you shortly.

I hope this helps, please let us know if you have any further questions.

    Hi Paul,

yes, that is correct, Jotform is available on MS Flow.

You can simply connect your Jotform account with Microsoft Flow, select the form you would like to use and then you will be able to send the collected data to Microsoft services like OneDrive, for example.

Here are the detailed steps on how to set up the integration, and here are some examples for what you can achieve with the integration: https://flow.microsoft.com/en-us/connectors/shared_jotform/jotform/

Let us know if we can assist you further with this.
