How do I disable the fields when the form is sent for signatures?

Hello, I have a form where I want my employees to complete and send to a customer for signature.  I want my employee to click on the link for each customer https://form.jotform.com/201494787228162 fill out the form, enter an email address where the form needs to be sent to for signature and send.  

Then the customer gets the form where all the fields are read only and all they need to do is sign.  

How can I accomplish this?
