No idea the form I used was violating your policies

I had no idea the form I created violated your policies.  I use it for clients when they make an appointment so I can get everything set up.  This form does not go out to the general public.   I really want to use your service it’s super convenient for what I do, what do I have to do to get this fixed?  I am a financial advisor and need to get all of that information from clients, birthdates and social security numbers are critical along with some of the general financial info, let me know thanks.  Thanks

You are right, your account was suspended since it was collecting sensitive information which is against our Terms Of Use.

You may collect these data but you have to add the T&C and Signature widgets.
