Conditions of form had been missing

Hi support team,

I'm in the trouble. The conditions I set was missing out of the form.

At first time, the condition had been missing so I had to edit it, and few days later It's been missing again (not same condition at first).

What should I do? Why the condition had been missing? In case someone edited the form but I don't know, how to track the editor?

I'm looking forward for your kind assistance.

To confirm, are you the only one who has access to your account? If not, I would suggest asking the people who you share your account credentials with if they have changed anything to your forms.

Alternatively, you can restore your form to a previous version where you think all the conditions are still there.

Hi Support team,

About the condition of form had been disappeared, it occurred again.

In this time, not only the condition was disappeared but also information of the form.

I have been asked my colleagues but no one edited the form and when I checked at history version it didn't show the condition and information which disappeared.

Why it unstable? Could you please help me to fix this problem?

Thank you for your response, the form I refer to is this form.

It's just some of the condition disappeared, such as the condition which I set to skip to another page, or the data disappeared with its condition e.g. I created one of the pilot course for user choose to skip to another page, and this time not only the condition but also that pilot course disappeared.

Do you have any solution for solve this case?

Thanks for confirming the form. Unfortunately, the only way to restore deleted/disappeared conditions is by reverting your form to a previous version.

Moreover, I see that you have more than a thousand submissions on your form so I'd suggest downloading your submissions first to avoid data loss.
