Name and DOB as one entry are not displaying side by side.

You have responded and helped me with a solution to how to get the name and DOB on the same line and I was successful with some of the lines but not others. I have followed the same process with each line but it is not working on 1/2 of them. See attached screen shot.

Have you solved this problem already? I did check your form "Updated Patient Information Form - Bay Street" and it seems to display the field correctly. The patient's name and DOB fields are being displayed side by side. Please check the screenshot below:

The form is the Updated Patient Information form. When I look at the form in preview setting it is still not lining up all the Patient's Name and Patient's DOB. Thanks

Hi, there are two forms in your account that contain "Updated Patient Information" text in their form title. Could you please confirm which on is it?

Is it Updated Patient Information Form - Bay Street

or

Updated Patient Information Form - Doctors Pediatric?
