submitting media to a form

    Hello.


    I am writing to explore the option of using jotform in my medical clinic. 

    I understand it is easily possible for the patient to fill a form prior to his appointment, and for me to update the form at the clinical examination.

    What I am particulary interested in, is whether I can store media onto his form (e.g a clinical photo or video uploaded from a mobile device), and how that would be viewable and downloadable later on. 

    1. Is video submission even possible?

    2. What interface would best fit my workflow in the clinic to achieve this (after taking a photo and video of the patient with my mobile device)

    1. Is video submission even possible?

    Yes, you can upload any file types including videos to our form. The maximum file size, however, is 1GB per file.

    We have more than 500 medical related templates for you to use to get started. 

    As for your "taking photo" requirement, you can use our Take Photo widget to achieve that.
