Scheduling emails to recepients

I need to send Covid screening form to my clients before they come to my clinic, 14, 7 and 2 days prior to their appointment. Is there a way of scheduling these forms to be delivered via jotform, if so is there an alert that would notify us if the client hasn't filled it out and submitted.
