    On many occasion and on several of my forms, the autoresponder email attaches a blank original form.  For some entries, it does not populate the field entries from the online GUI to the original form prior to emailing or saving it in Google Drive (I have the integration set up).  Is this a known issue?  

    The work around is that I have to go in and edit the submission and submit it once again.  It usually takes the second go round, but having to do that almost defeats the point.

    Hi there,

    Thank you for reaching out.

    I went and did a test submission on your form and I couldn't seem to replicate the issue as all the data entered did appear on the attached PDF in the auto-responder.

    May I please confirm if the issue still exists on your end? I also went ahead and cleared your form cache on our server.
