New patient registration forms

I have a small pediatric private practice and am trying to redo my patient registration forms. If I use the Jotform, can I print out the form and give it to patients in the office to fill out. Once the forms are filled, are they in a printable format to scan and enter into the EMR?Can multiple front staff members access the forms that are printed?
