Embed form disappeared from my site

this was the code that i was using before - now it simply has disappeared

I have even deleted the page and put a new page in and re input the code but to no avail

I've checked your form and you have form ACCESS SETTINGS set to Private, hence it's only available to invited people. If you want it to be accessible in embed website as well, you need to switch that setting to the Public as shown below:
