My Account was suspended while creating a form

Hello I just received a notice doing my process of creating a form that my account was suspended. I did not ask for any sensitive information on my last for creating. I own a trucking dispatch company and the info I request is pertaining to our carriers who has signed an  agreement for us to book freight loads for them.

Regardless of its type and usage, we do not allow it. It is against our Terms of Use (https://www.jotform.com/terms/). 

I have reactivated the account, but I have deleted the said fields. Please refrain from adding the fields again.
