How to make a PDF fillable form editable?

    I have two forms that I am using the pdf converter.  The form text did not fully pull over to the editable format and stuck on how to make the editable version read like the original pdf.  Second, these pdfs need to be completed in this form https://form.jotform.com/KLiebs/16th-annual-mobile-health-clinics-c

    I am stuck on how to get these pdf's in this form. Here are the two pdf's I have been working on editing to insert in the form above:

    Kindly note that the PDF converter tool will import the fillable fields, some fields may not be identified by the PDF converter and some static text may be missing as well, but it's possible to add the missing fields and text using the elements the builder provides, you may add any fields and text using the "Add Form Element" option.

    I am stuck on how to get these pdf's in this form. Here are the two pdf's I have been working on editing to insert in the form above. 

    I'm afraid that this part is not clear, do you want to connect the PDF forms with the web form? 

    If so, that's not possible, unfortunately, but feel free to provide more details so we can understand your question better and will be able to assist you.

    If you look at this form,  I originally put these editable PDF forms in here, but it would not let someone save it.  That is when tech support suggested I created this form in the PDF converter.

    This is a previous ticket where you asked how to embed a fillable PDF document into a web form, based on my colleague response, now that you have the PDF fillable forms imported, you can embed them using the iFrame widget, as suggested on that ticket, here is the link to that widget: https://www.jotform.com/widgets/iframe-embed

    So basically you need to add two widgets to your web form and then use the direct links of each form on each iFrame embed widget accordingly, to get your form URL you may refer to this guide: https://www.jotform.com/help/401-Where-to-Find-My-Form-URL

    I will give this a try.  I was thinking it was not possible based on one of the thread messages.  Thank you.
