Submitted Form is Not Equal to What Was Built

My "Name" built into JotForm comes out as "Buyer's Name" on the submitted form.  They are also flip-flopped on the submitted form.  

My plant also is under "company name" on the form but show under the address on the submitted form.

I believe you are referring to the form "90483886812165".
