Form Disabled, no details on account page

    Hello,


    My form has suddenly become unavailable. It was built in the past two weeks and has been live since last week, during which time it worked and I was able to collect responses. I just noticed the form can no longer be submitted. After filling in responses, users encounter a page indicating the form has been disabled.


    I have not disabled it and I see no errors on the administration page so what has happened? I see in the forums that forms are sometimes blocked due to violations but I was not informed of anything like this and do not see it reflected in my account. Please help!

    1. Go to https://www.jotform.com/myforms

    2. Restore the form in the trash folder
