Why my form is disabled?

Hi, I cannot access my form. It seems my ID is not allowed? Or form was disabled? Can someone assist please. Thank you.

One of your forms is asking for login information which violates our terms of use. That's the reason why your account got suspended or your form is disabled. As per our terms, we do not allow collecting login information here in JotForm. Also, the word "Password" and "Username" will get your account automatically suspended because of our anti-phishing detector tool.

I have now deleted the offending fields from your form and activated your account.
