checkboxes wont click

its been about 4 hours since i submitted this issue. Let me try again. 

On my form the  "purchase order" has stopped working seemingly by itself.  Checkboxes won't click.  Thi is an important form for my business to sell PPE.  Sent out the list to 6000 people and now I've had to disable the form.  To say I am disappointed is an understatement.  DId i mention its been 4 hours with zero responses?
