We apologize for inconvenience. I test your form on the website using mobile browser and I was able to see the issue as well.

What I suggest for now is to re-embed the form in your website using instead its iframe embed code.

Guide : Getting the Form iFrame Code

Let us know if issue persist even after doing this already.

I checked the website and the embedded code is still the default. Please try using the iFrame embed method> https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

Also, the issue has been forwarded to our developers on this ticket: https://www.jotform.com/tr/answers/2361770

We will contact you on that ticket once we have any updates. Thank you.
