Suspended account reactivation request

    Hi team,

    I get the attached feedback when I try to look into my account, kindly review and assist with reactivation as soon as possible.

    I have checked the account related to your email address, but this account is not suspended. If you are having problem with some other account, please provide us the username or email address of the suspended account.

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting ID numbers is considered as phishing activity, as stated in our Terms of Use.

    Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.


    Your account has been suspended due to the usage of copyrighted logo on your form. In order to reactivate your account, kindly provide us the following information:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #2362153 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
