Required fields being bypassed ocassionally

Please assist us with the following issue on one of our forms.

On this form we have a required "Disclaimer" field, which the person need to select to submit the form. Yet on the submissions page and the downloadable excel form it seems that in some of the registrations the "Disclaimer" field was not clicked, how is this possible? (Please see the screenshots below)

I suspect this has something to do with your form's cache so I cleared it from our end. 

Moreover, I can see that you have multiple errors in your conditions. These errors can cause your form to misbehave. 

Kindly remove/update your conditions to eliminate the errors.
