New field added to the form is not being displayed

Greetings:

I tried to add an input field to my form. In the designer it shows up. However, in the live version. It is not showing up.

Can you help me fix this please?

This is happening due to the conditions set on the form to show the City field based on another field.

Please check the conditions on the form and delete the condition to show the City and the other missing field so that the form fields should be displaying properly on the form.
