Account suspension

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. The form 'Password cheat sheet' form in your account has triggered the system to suspend the account. It is not allowed to create login forms or collect sensitive information like passwords, credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    In order to activate your account, it is required for you to agree with our terms of usage and agree to delete the guilty fields on the form. Please confirm so that we can delete the guilty fields on the form and activate your account.
