The input for Full Name has 3 separate fields, one each for First, Middle, and Last Name. However, the PDF output lumps them together into one field. Can I have 3 separate fields output in the PDF?

We apologize for inconvenience. I think there is a misunderstanding about the Full Name field.
Although there is option for multiple input i.e. LastName, MI, FirstName, the field itself is only one and not 3 fields each.
This is the same in the submission PDF document as well.
If what you need is separate fields for each element (last, mi, first), you can use the Short Text Entry field instead for each. The result in the PDF document will also have 3 different fields for each element.

Got it, thanks for clarifying.
Could you help me with CSS codes to fit First, Middle, and Last name on one line with all fields equal width? I set character maximum to 40 for each of the fields. I might need a code for the @media query to make sure everything lines up properly (I needed it for the address fields that I adjusted the size of)Thanks!
