Add a date field to the PDF Output

Hello! I need to add a "date" entry in a survey I already completed. How can I insert it in the first question?

If you already have submissions in your form and a new field is added, just the new submissions will have the new field, the old submission is going to maintain the data but not the new field data.

The only way to add a new field information is with editing the submissions:

But I need to edit the ORIGINAL form not the submissions. Can I do this? I won't be able to add the date on every submission from every patient. It is not feasable for me.

After checking your forms I cant identify the form with the original PDF but if you are using the PDF Editor you can add the submission date to your output PDF:
