Reactivate my account

    Please help me to reactivate my account.

    You can review my account, my forms adhere to our terms of use, so please reactivate my account. My form’s purpose is not to deceptively obtain sensitive personal information, such as credit card information, social security numbers, user login credentials, and driver’s license numbers, etc.

    Your account was suspended since the forms 201494589688475 & 201478840040045  on your account were flagged as offering fake credit services.

    We understand that this may not be the case, but we had to act according to our Terms: Free accounts with free domain, offering services without a valid domain on their registered JotForm accounts are suspended immediately.

    In order to reactivate the account, we will need to delete and purge these offending form/s.

    However, if you do not want us to delete the offending form/s, please provide the following information:

    c.1.  The URL/website where your forms are used.

    c.2.  Provide any document that will support your authorization to offer credit services.

    c.3.  Your website or proof of affiliation to a legitimate institution.

    We will also need you to change your account email address to a corporate/business domain address.

    Send all of these proof to reviewers@jotform.com with the Thread #2364494 as the email subject. We will get back to you about the status of their account after a thorough review of your  proofs.
