Im trying to integrate jotform contact form on my website

Hey I have the same problem with my website. I noticed the that the add form button is not appearing on my website. Please help me out. How should I embed from this point? And still when I import the form from my jotform account, I test it the submit button is stuck on Please wait... what are the possible errors that might have occured for this to happen?
