    The user wants to distinguish current or past for each condition. What I have is just a multiple choice boxes. Is it possible to have two columns (current and past) of multiple choice check boxes?

    Hi, we have a new field that you can try. It is called Fill in Blanks. Example:

    1591225625fillinblank.gif

    If you preview it, it will look like this in the browser:

    But, you can shorten the text fields by injecting this CSS code in your form: https://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes

    li[data-type=control_inline] .FITB-inptCont[data-blottype=textbox] input {

        width: 20px !Important;

    }

    Example:

    1591225820fillinblankcode.gif

    Hope this helps.
