    Hi. How do I implement this different options in my form?

    The user wants to distinguish current or past for each condition. What I have is just a multiple choice boxes. Is it possible to have two columns (current and past) of multiple choice check boxes?

    Hi, we have a new field that you can try. It is called Fill in Blanks. Example:

    But, you can shorten the text fields by injecting this CSS code in your form: https://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes
