How to remove the word JotForm.com in the web address

Once we start paying for service, the JotForm logo will no longer be visible in the lower left corner. I understand that.

What steps need to happen to not have the word jotform.com in the web address?

Our users will be clicking on this link from our website, and when they are bought to the form, it should still look as though they are in our site. I think this can be accomplished if jotform.com is not in the web address.
