Trouble with account

    I downloaded the app, created my account and completed verification through a link my employer sent me but after I created the account, I was notified from my employer they're having a problem with the form and they asked I verify on my laptop first then download the app.  So I delete my account and now I am stuck.  Can you send me the steps to fix this problem. Thanks.

    May you please clarify a bit the issue you're facing? 

    Is it with the form or do you want to reactivate your account?
