Still cannot get workable URL generated for prepopulated form

    PLEASE help me rectify this issue!


    As you can see this issue is still not solved and you can see multiple parts of the form are errant or missing. The payment widget doesn't show up at all, the check boxes don't show up, and of course the widget for authorize is still a problem.

    I see you are using the Polaris Checkbox. Could you please switch to the native checkbox and let us know how it goes.

    The field Monthly payment could not be calculated as you have an equal to sign in the syntax:

    May we know what issue are you experiencing while authorizing the payment.

    I removed the plus sign above as requested and took a screenshot too which is attached here showing that the math calculations totally fail on prepopulate and the checkboxes are now dropdowns. 

    PLEASE HELP ME RESOLVE THIS TODAY!

    ALSO THE Ach info - not sure how to connect that through Authorize.net. Please advise.
