I created three conditions based on whether a user selects 1. Design and Print 2. Promotions of Event 3. Series Marketing
Here are the issues I have:
1. When the form first comes up I don't want the field that says Design and Printing. Complexity of design.... to so. It should only show if they select Design and Print.
2. When they select Design and Print all the correct fields show up. If you select 2. Promotions of Event or 3. Series Marketing it leaves off several of the fields I told it to show in the conditions. i.e. Full Name, Email, Phone, POD or department, Campus, Notes and Submit.
Can you not use the same field for multiple conditions?

If you'd like to show the same fields "if any" of the three options is selected, I would suggest setting up the conditional logic like this to avoid conflicts:
I also suggest using Form Collapse Field to show a multiple fields at once to avoid such long conditional logic. This guide: How to Hide or Show Multiple Fields at Once Based On a Single Conditional Entry.
I've check your form, and there are few number of fields, and I can't distinguish which fields belong to a certain option and which are not. Can you please let enumerate this to us so we can further help?
These fields should only show if "option 1" is selected. and.....
These fields should show if "any option" is selected.
