Draw on form doesn't display on chrome or safari on my iPad-which is pretty old I'll admit.
It works on the iPhone 6 but we need to use it on a tablet.
I haven't tried it because I don't have one here.
iOS 9.3.5
Thanks, I'll try and find my little android table but ut is almost as old.

May we know the IOS version of your iPad?
It could be that some scripts may be not be supported in your iOS or safari version.
Looking forward for your response.

Thanks for the details.
I have been testing your form and here are my results:
- Using an iPad emulator with iOS 8:
- Using an iPad emulator with iOS 13:
It seems like this is a compatibility issue with the old version of iOS, I would recommend you to kindly test with a recent device and see if the problem persists.
