    Accidentally locked myself out by using wrong password to many times.

    I have checked your account and this was suspended due to the insurance forms on your account, the suspension was triggered since your account is registered under a free email address which did not allow the corresponding team to verify your activity. 

    In order to reactivate your account, we will need to get some sort of proof about your business, you may either provide us any documents or email us from a corporate email address (i.e: user@mycompany.com) to support@jotform.com, you can also use this ticket ID on the subject as reference: 2369164
