PDF form downloaded is not usable

I've edited this form a few times and even cleared my form cache, but the PDF generated by the Publish - Download PDF option is unusable.  Most of the signature boxes are missing, one shows a signature when it should be a blank form.  Also, several fields are out of order.  

Please go to your PDF-editor to customize your PDF form.

Also, the signature field added to the PDF form is not the fillable one, please remove it and replace it with the correct signature field.
