Hello,
I am receiving duplicate entries for each request submitted. Are the people submitting the request doing something wrong or is this a glitch?

Hi there - Apologies for the inconvenience.
I have checked your submissions and it seems not all of it has duplicates. I suspect some of your submitters are refreshing the page causing duplicate submissions.
To prevent this from happening in the future, kindly disable Browser AutoComplete on your form so when the form is submitted and the page refreshes, the fields will then be blank.

Apologies for the inconveniences caused.
I've been checking your form and this seems to be configured to prevent the browser autocomplete. I can see you have one submission at the moment there, but would like to ask permission to test the form.
Also, is your form embedded on a web page?
If so, you may share us the link, so we can test it further, another thing I would like to suggest is adding the Get User Agent widget to your form, this way we can also get the browser details where the users replicated this issue, here is the link to the widget: https://www.jotform.com/widgets/get-user-agent

There is only 1 submission because I erased the previous submissions and submitted 1 as a test.

Based on the screenshot you have shared, you are receiving duplicate submissions. Since you have already deleted the submissions from your form, we cannot check if the submission was received form same IP and its timing to analyze if the duplicate submission was sent intentional.
For now, I would suggest you to please add "Captcha" field in form which will prevent form resubmission if they are resubmitting form accidentally or refreshing the thank-you page. Please check the screenshot below how to add "Captcha" field in form:
Please monitor your submissions after adding captcha field and see if that solves your problem. If you continue to receive duplicate submissions, that means someone is manually submitting form again. We will explain to you how to prevent duplicate submissions in form.

Hi,
The duplicate submissions problem is now resolved. We added some preventive measures to help deter unwanted resubmissions of the previous entry. This should not happen again.
