Trying to go paperless as much as possible. Want to create a form that will
be a client intake form and a form for consent/release because of Covid.
Are your forms capatible with MindBody software? Meaning can I upload into
my Mind Body software. Don't know if you are familiar with that software.
Also, how much are the forms?

Unfortunately, we do not have a direct integration with MindBody.
However, there are some online services, which offer this integration (JotForm+MindBody). For example, LeadsBridge. You can read more about it here:

If you want, we can submit a feature request for direct MindBody integration. But there is no assurance that this will be implemented.
