My forms keep renaming themselves

Every time I change a form name it keeps renaming to the first form I created. How do I stop this it is very irritating. 

I see that there are 7 forms in your account and they all have different form titles. Could you provide us with more information with the form ID and a screenshot so that we can assist you further?
