Add infinite fields dynamically

I would like to create a form that has the possibility to dynamically add fields in it (as per screenshot) is that possible?

There is section on styling the List as well in the above mentioned guide. Though it is not very detailed but looking at the code in it will give you a good idea. A Developer can easily figure out how to apply styles to this List by just looking at the generated HTML code.
