HIPAA: Forms keep renaming themselves.

    This is a formal complaint that I would like to submit to the technical team. 


    I have seen this issue on other forums and need to know if this is an ongoing issue. I run a business and cannot use these forms If they will not stay intact. 


    My first issue is that every time I try to rename my forms it reverts to the name of another form on my account. I have edited the title in multiple places and it will not save. I have used both chrome and firefox. 


    My second issue is that when I attach a PDF through the PDF generator tool and add fields to it, i cannot get the original PDF to send in the autoresponder email. When I go into the form setting and check this box, it reverts back to being unchecked even after I put in my password for the form and click save. 


    What needs to happen for this issue to be resolved? Otherwise, I will stop using jotform as it is not reliable.

    Hi!

    Are you referring to the Form name (also shows up as the Page Title). You should be able to change the Form name from "Form >> Settings >> Title" (as shown in the screenshot below). If you can't, then please provide us the Form IDs or Form URLs with which you face this issue. We'll take a look.

    Looks like you tried to send us an image. If so, we have not received it. Please use this guide to attach screeenshots to your replies on JotForm.

    Also, please provide us the Form ID/Form URL with which you're facing this issue.

    * Every time I create a new form, it reverts to title employee offer letter w2
    * I have tried changing the name on the forms dashboard, in the forms settings, and in the form builder at the top where the form title is located. I have done this on two separate browsers. This issue has effected other forms too which replaces them with the same form name indicated above.
    I didnt try to send you an image. The problem still persists
    why are the PDF boxes not remaining checked after they are saved to attach to the autoresponder email?
    * Form>Settings>Form Settings Tab>Enable original PDF
    * This will not stay checked!! If you check it, create and save the password and go to another tab then come back, the box will be unchecked.

    Apologies for the inconveniences caused. 

    Unfortunately, it's not possible for us to log in and check your forms due to the HIPAA compliance; however, we will be happy to assist you. 

    May you perform some tests using incognito mode? 

    It's possible that the browser is auto-filling the form's title, JotForm does not have this feature so we need to make sure there is not an external factor causing this. 

    Please let us know how it goes and if the issue persists using incognito mode.
