Jotform - Letter of Intent for Promotion

    Hi - Our organization has an account with Jotform. Today, I noticed that there was a Jotform titled "Letter of Intent for Promotion" dated May 29 in file. We are a staff of 2 and an IT consultant and I inquired if anyone created this form. No one on our team did. How can this happen? We have an unusual password, so I can't imagine that anyone outside our team would be aware of this. Do you have any ideas or can you tell who might have created this form?

    Thank you for your message.

    After checking in your account, I found that the form has been added to on May 29th, I was curious why there is no history of changes, it is because the form has been added from one of our Templates, this one https://www.jotform.com/pdf-templates/letter-of-intent-for-promotion.

    Once someone that is logged in your account click on "Use Template" that form is added.

    I search under our internal logs and we didn't find any support rep accessing your account, and we don't have any different IP addresses in your login history, you can see the login history here https://support.jotform.com/myaccount/history.

    So we can say that maybe it was a mistake/misclick.
