    I recently made the profile and forgot login info, and I forgot what they were. Now when I log in it says I am locked out. I set up an application for our company and don't have access to set up email or anything like that. Please let me know what to do asap. Thank you.

    Upon checking, below are your form violations.

    1). Loan / Credit application form — A form offering loan/credit must be done from authorized bank or credit unions only with company email address on your registered JotForm account. 

    2). Asking for SSN

    3). Asking for Drivers license.

    For #2 and #3, 

    For forms asking this type of information, you need to comply with two things.

    1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

    2. Add a Signature widget where your users sign and agree to provide such sensitive information.

    What we can do is delete the offending fields and reactivate your account if you agree to our terms https://www.jotform.com/help/8-Terms-of-Use.

    Yes please delete them, I wasnt aware. I was looking for it to be more
