My form was replaced with a testimonial form

I created a form, “Destiny for Dollars”, and after four submissions, the form was turned into another form that I did not create. See attached.

I checked your form and the form name on the browser tab and on the form are different. Probably that is why you believe that the form was changed.

You can update the browser tab name from the Form settings of your form by changing the page title as explained here: How-to-Change-the-Page-Title-of-the-Form 

If you think that the form content has changed, then you can revert your form to a previous saved version: How-to-View-Form-Revision-History
