Reactivate my account

My account has been suspended for Creating a Pubg tournament Registration form and asking for people's information which is avaliable publically! Please look at my form and reactivate my account.

Will it be suspended again for the same reason if I create another form
like that?

Please note that we have automated anti-phishing filter which scans all forms. I would suggest you to please do not use any keyword like username, password, credit card or any other field to collect sensitive info and you should not have any trouble.
