Why are my forms showing up disabled?

When i click on my form the screen says that the form is disabled but they aren’t. 

I see you have this empty form https://www.jotform.com/form/201574573830052 which kicks off our system to check and auto suspend your account. It does look suspicious.

I have lifted the suspension. Please delete the form or add fields to it.
