Is it possible to add other users (doctors) to the JotForm app so they can fill out the medical evaluation forms directly in the app? Patients too?

You can create a workflow for medicals and patients to fill out, sign and submit the same form. In order to do that, you will have to use some of main features, like conditional logic, hidden fields and email notifications.

Maybe it does not meet all your requirements, but you could easily adapt it to your needs.
