The widget "Take photo" is not working.

I would like to inform you that the widget "take photo" is not opening either with IOS or ANDROID. Can you please check it?

Upon checking your form, I can see that there indeed seems to be an issue with the  "Take Photo" widget.

Can you please try removing the widget and re-adding it?
