Why is my account suspended?

I removed tax pdf form from my account but it seems my account suspended again.

Could you please help me?

I did check your account and found that it is currently suspended because of the driver's license file upload field you have added in form "Driver Application Form". Please check the screenshot below:

You may collect identification numbers only, however you have to add an agreement displayed on the form and a signature, widget fields.

Can I collect driver's license photo including Terms & Conditions and digital sign widget?

You can request for ID uploads but ensure that you added the mandatory widgets as well. And that you are collecting these to your customers only and not from the general public.
