Can I Create Exactly This Kind of Form in Jotform

    I like this kind of layout, tried to create like this but not getting success. Can you help me?

    but not found.

    Sir, can we edit and add more fields similar to in the form. Because I am trying to add more drop-down but it changes its shape. It has not a similar shape like already placed fields.

     

    It has a smaller header like and with a different color.

    The form you have cloned has custom CSS codes specifically made for its existing fields. This is the reason why it does not affect to your newly added fields.

    I would suggest, however, to proceed with adding your own fields and we'll help you with the CSS codes afterward.

    Once you have added all your additional fields, let us know the URL of the form you're working on and we'll be happy to check it for you.
