When this form was filled, an email with the filled-in information was sent to you under your Email Alert (Notification Email).

However, the person who filled your Form didn't receive any email. This is because you're not collecting their email address on your Form. If you want your users to receive an email as well, please create an Email address field on your Form and set your Autoresponder to send the Email to the Email Address provided under this particular field.
