My account got suspended for no reason

So im logging in to put a survey in for my discord server but my account said i was suspended. I dont know what i did on anything on my surveys that was sensitive but please unsuspend my account

I would like to inform you that, your account has been suspended for requesting Login information in your form named Heading. Due to our Terms of Use, the gathering of certain private information such as Driver's License information, Credit Card details, Login Information, Social Security Information, etc. is not allowed without the mandatory widgets.

You need to include E-Signature widget in your form. However, we also require that you include a Terms and Conditions widget with your terms for your users to read.

I went ahead and removed the offending fields from your form and reactivated your account. You are more than welcome to revert the changes in your form so the fields are restored and kindly add the necessary widgets so you do not get suspended again.
