    I apologize for the inconvenience. I believe this form got disabled because it is collecting ID numbers, without having a Signature Widget.

    Please note that this is against our Terms of Use. Kindly give us permission to remove this field so that we can enable the form again.

    Hi,

    I have removed the ID Number field of your form, and the form should be available now.

    I can see that you already have Terms and Conditions on your form, which is great, please add a Signature Widget as well, where the person filling out the form can agree to provide sensitive information (ID number in this case).
